Free Sports Physicals will be held Tuesday, May 21 from 5:30pm to 8:30pm at Harrison County Hospital
Local primary care physicians donate their time to offer free sports physicals for student athletes entering grades 7 thru 12, who live in Harrison or Crawford counties.
The Harrison County Health Department will also be on hand to administer free Tetanus, Menactra (meningitis vaccine) and Varicella (chicken pox) vaccines for those with Medicaid or no insurance. Consent forms must be signed by parent to receive the vaccine.
Parents, please bring an updated shot record for your child.
APPOINTMENTS ARE REQUIRED! Appointments must be made by calling
812-738-7869 between the hours of 8:30am - 4:00pm ONLY on the following Tuesdays -
Tuesday April 16 - Tuesday April 23 - Tuesday April 30 or Tuesday May 7
Appointments are on a first-call-first-served basis. Appointments will be taken until the
schedule is full. Messages will not be taken during appointment scheduling. If you get a busy signal, please call back. (When appointments are full, a message will be left on the line.)
Athletes must bring their IHSAA Pre-Participation Physical Evaluation Form, with parent and student sections completed, the completed Vaccine Consent Form and current shot record if vaccinations are needed.
