Health Services: Enrollment Information for
Grades K-1
- Indiana law requires that all children
newly enrolled in Kindergarten or Grade 1 and less that 7 years of age
be immunized against DIPTHERIA, PERTUSSIS, TETANUS, POLIO, MEASLES,
MUMPS, RUBELLA, HEPATITIS B AND VARICELLA (Chickenpox). If your
child has had Chicken Pox, we need a note from the physician stating the
month and year of the disease.
- No child will be permitted to
attend school unless he or she is fully immunized or can show evidence
that immunizations are in progress. If immunizations are in
progress, a schedule for completion of immunizations must be sent to the
school health office.

- Minimum Immunization Requirements:
- 5 doses of Diptheria, Tetanus,
Pertussis (DTP) or DtaP or DT
- 4 doses of Polio vaccine, OPV
or IPV
- 3 doses of Hepatitis B vaccine
- 2 doses of MMR (Measles, Mumps,
Rubella)
- 2 doses of Varicella
(Chickenpox)

- An official birth certificate, issued
by the state or county health department in which your child was born,
is required prior to school attendance. HOSPITAL
CERTIFICATES ARE NOT ACCEPTED. The school office does have
birth certificate application forms and information.

- *Please note- your child will
not be enrolled and will not attend classes until immunization
records and birth certificate are received in the office.

- Consult with your child’s physician
regarding your child’s immunization schedule. Immunizations are
given without charge at the Harrison County Health Department at
738-3237. A tuberculin test and HIB vaccine are recommended. Please
ask your child’s physician about the need for these.

- Your child’s physician and dentist may
complete the attached student health history. This form should be
returned to the school on or before the first day of school. A complete
eye/vision exam is strongly recommended for your child to
identify vision problems or diseases of the eye.
